This form must be completed and acknowledged prior to participation in any training, daycare, boarding, or board-and-train services with The Everyday Canine, LLC.

Vaccination & Health Requirements (REQUIRED for ALL SERVICES)

I CERTIFY AND ACKNOWLEDGE that my dog meets, ALL of the following requirements to participate in any of the services with The Everyday Canine. Including training, daycare, boarding and train-and-board.

My dog is current on all required vaccinations, including BHLPP (Distemper/Parvo combination), Rabies, Leptospirosis (Lepto), and Canine Influenza.

My dog has had a negative (clear) fecal test for Giardia within six (6) months of their scheduled appointment.

I understand that proof of vaccinations and fecal testing must be provided prior to participation and may be requested at any time.

I understand that failure to provide acceptable documentation may result in denial or removal from services without refund.

I certify that my dog is free from contagious illness at the time of attendance and agree to notify The Everyday Canine if symptoms arise before or after services.

Training Philosophy & Tools Consent

I understand that The Everyday Canine utilizes a balanced, humane training approach tailored to each individual dog.

I acknowledge and consent to the responsible use of training tools when appropriate, including but not limited to:

● Flat collars

● Slip leads

● Long lines

● Harnesses

● Prong collars

I understand and acknowledge that remote training collars (e-collars) are NOT used without my explicit approval, and that separate consent will be obtained prior to any e-collar use.

Daycare / Boarding / Training Expectations

I understand that participation in daycare, boarding, or training is not guaranteed and depends on my dog’s behavior, stress level, and safety on any given day.

understand my dog may rotate through play, rest, enrichment, crate time, or one-on-one handling as determined by staff.

I understand that rough, unsafe, or disruptive behavior will be interrupted and participation may be modified or discontinued.

Client Communication & Operational Boundaries

Acknowledgment

I ACKNOWLEDGE AND AGREE that I have reviewed The Everyday Canine’s Client Communication & Boundaries Policy (provided via hyperlink), including but not limited to:

● Approved communication methods

● Business hours (Monday–Saturday, 7:00 AM – 7:00 PM; closed Sundays)

● Designated pickup and drop-off windows (7:00–10:00 AM and 4:00–7:00 PM)

● Update expectations for daycare, boarding, and board-and-train services

● Respectful communication standards and response-time expectations

I understand these boundaries are required to ensure consistent care, safety, and daily operations.

Assumption of Risk & Full Release of Liability

I understand and acknowledge that participation in dog training, daycare, boarding, and board-and-train services involves inherent risks, including but not limited to injury, illness, behavioral incidents, escape, property damage, or accidents involving my dog.

I voluntarily assume all risks associated with my dog’s participation, whether known or unknown, foreseeable or unforeseeable.

I expressly agree that The Everyday Canine, LLC, its owners, members, managers, employees, contractors, agents, volunteers, insurers, landlords, and affiliates shall not be held liable for any injury, illness, loss, damage, or incident of any kind occurring while my dog is in their care, custody, or control, including but not limited to:

● Injuries caused by other dogs

● Self-inflicted injuries

● Illnesses or exposure to disease

● Stress-related behaviors or behavioral regression

● Escape or damage to property

● Accidental injury or death

This release applies regardless of cause, except in cases of gross negligence or willful misconduct as defined by applicable law.

I further agree to indemnify and hold harmless The Everyday Canine, LLC from any and all claims, demands, damages, losses, liabilities, costs, or expenses (including attorney’s fees) arising out of or related to my dog’s participation in services or actions while in their care.

Veterinary Emergency Authorization

I authorize The Everyday Canine to seek veterinary care, including emergency treatment, if deemed necessary at their reasonable discretion.

I understand that I am financially responsible for all veterinary expenses incurred.

Owner Responsibility & No Guarantee of Results

I understand that training and behavior results are not guaranteed and depend on owner follow-through, consistency, and environment.

I acknowledge that behavior regression may occur if training protocols are not maintained after services conclude.

Media Release

I grant permission for photos and/or videos of my dog to be used for educational, promotional, or marketing purposes by The Everyday Canine.
